If the insured person is alive at the time, the insurance contract expires::
- insurance certificate;
- a statement in the prescribed form;
- a document proving the identity of the insured person.
In case of death of the insured person during the insurance period:
- insurance certificate;
- statement in the prescribed form;
- a certified copy of the death certificate of the insured person;
- other documents necessary to determine the cause and nature of the insured event;
- notarized copies of documents proving the identity of the recipient of insurance and, if necessary, a certificate of inheritance.
- intentional action or inaction of the policyholder (insured) in the event of an insured event occurred, with the exception of cases provided by law, as well as a deliberate crime directly related to the insured event;
- if the cause of the event is the result of circumstances that are not covered by insurance, unless otherwise provided by the contract;
- intentional failure to take measures by the policyholder (insured), if he is able to take the necessary and possible measures to prevent or reduce damage; In this case, the insurance payment may be reduced to the extent that the insured has reduced the amount of damage provided that he / she has taken reasonable measures;
- if the policyholder (insured) prevents the insurer from investigating the causes of the insured event;
- if the insurer is completely or partially deprived of the opportunity to assess the insurance risk, as well as to determine the causes and (or) the degree of damage as a result of the deliberate distortion of information by the policyholder (insured);
- if the accident is not considered an insured event under the law or an insurance contract;
- other cases prescribed by law.

- insurance claim of the beneficiary (s) to receive an insurance payment;
- a copy of the identity card of the insured person, with the exception of the death case of the insured person;
- a copy of the loan agreement certified by the lender;
- a copy of the loan repayment schedule certified by the lender;
- a document issued by the authorized body and confirming the fact of the insured event and (or) the reasons and results of the events, which can be considered as insurance events that need to be investigated or recorded;
- the original or notarized copy of the conclusion of the Medical and Social Expert Commission on the designation of disability to the insured person in case of disability;
- a copy of the death certificate of the insured person in the event of his death;
- medical certificate of cause of death; in case of death from an illness, an epicrisis and an extract from the medical history are also required;
- if necessary, a notarized copy of the certificate of inheritance.
- If the occurred event is not considered insured one in accordance with the following:
- suicide of the insured person during the first 2 years after the conclusion of the insurance contract, provided that he was not forced to commit suicide as a result of unlawful actions of third parties;
- the active participation of the insured in hostilities, civil conflicts, military coups and riots;
- terrorist acts using nuclear, biological and chemical means;
- illegal actions of the insured directly related to the event, which can be considered as an insured event;
- alcoholism, drug or toxic intoxication of the insured, including driving in a state of intoxication;
- consequences of nuclear explosions, radiation and radioactive contamination;
- the crash of an aircraft managed by the insured (including hang glider and parachute);
- if ensured person is occupied with winter, underwater, equestrian, aviation and combat sports, auto-moto sports, parachuting, rock climbing, mountain climbing, hang gliding, mountain and water tourism.
- payment of any other part of the insurance premium within 15 days after the expiration of the payment period specified in the contract;
- if the insurer is completely or partially deprived of the opportunity to assess the insurance risk, as well as to determine the causes and (or) the degree of damage as a result of the deliberate distortion of information by the policyholder (insured);
- the deliberate action or inaction of the policyholder that led to the accident, as well as the deliberate commission of a crime directly related to the insured event;
- other cases provided by law.
- a copy of the insurance contract;
- insurance claim signed by the insured, beneficiary, in case of death of the insured person;
- a copy of acts confirming the fact of an accident at work or a court decision drawn up in the manner prescribed by law;
- copies of extracts from examination certificates provided by the Commission for Medical and Social Expertise that determine the degree and duration of disability of the insured person (indicating the cause of disability and the percentage of the degree of loss of professional ability to work);
- a copy of the death certificate of the insured person in the event of death;
- in case of death of the insured person, a certificate of family members (persons) who were under his guardianship;
- a decision of the Commission for Medical and Social Expertise, confirming the death of the insured person in connection with an accident from which he was insured and paid a monthly insurance cost;
- copies of the identity card of the insured person and other beneficiary in the event of his death;
- copies of the birth certificate of the insured person, in case of his death, if his parents are beneficiaries, his children’s birth certificates, if his children are beneficiaries, or marriage certificate if the beneficiary is the spouse of the insured person;
- beneficiary's application for insurance payments and bank details;
- a written statement by the employer about the average wage of the insured 12 months before the accident and certified by the employer;
- as a result of the intentional actions of the insured, leading to an accident;
- if the accident occurred as a result of the insured being in a state of alcohol, drug intoxication or under the influence of psychotropic, toxic and other poisonous substances in the performance of his official duties;
- If there are other general grounds for refusing insurance coverage, prescribed by law.
